She was born and raised in the San Francisco area and began her modeling career at age sixteen.
She first acted in the 1990 film Wild Orchid, which starred her future husband, Mickey Rourke. The film contained a sex scene between Otis and Rourke that was rumored to have not been simulated.
She was married to actor Mickey Rourke from 1992 to 1998. In 2005, she married Matthew Sutton.
Otis suffered from an eating disorder during her modeling career that was so severe that it resulted in damage to her heart. Actress Rie Tomosaka also publicly admitted struggling with an eating disorder.
